During a climbing session with our university club the injured person was down climbing on an easy boulder, their foot slipped and they slid off the wall, they landed with their foot awkwardly between the mat and the wall which caused their ankle to snap.
Lessons
The climber was a novice who was being closely supervised, knew to land with their knees, and all precautions were taken to prevent injuries, unfortunately this seems like the situation that is hard to prevent in a high risk sport
