Incident
My partner reported (with video evidence if needed) that one of the anchor points (p- bolt/hanger) was very loose and infact the nut was almost completely undone. Partner did the nut up as tight as he could by hand and used another anchor point.

Nut needs to be tightened to ensure that it doesn't come off during a climb causing injury
