I was belaying and my partner was leading a VS route. They had placed a tricam as the first gear placement and were doing a tricky move above it at around 10ft height. They fell and decked out as the gear popped out of the crack. Luckily they jumped back onto a soft landing so were unscathed.

Lessons

Be aware that gear can fail at any time. Ensure good placements.
Always wear a helmet- we were wearing helmets and my partner thinks this did protect the back of their head
