Climbing short route. Really bouldery start made me pumped. Got to the gear which was a wet slimy chossy crack that was not going to hold a fall. Tried to find more. Said take to my belayer who took the rope too tight pulling my already bad gear straight causing a possible walk of the cam. Lobbed and the cam ripped. Hit the deck and rolled.

Placements you can see from the ground aren't always as good when you get to them and taking in slack isn't always the best option.
