Hold span. Was fine when used as hand hold, as soon as foot placed on the hold, it span and participant fell with knees at height of first clip. Hit hold with knee while falling. Belayer caught fall but participant ended up 0.5m from ground

Lessons

The hold should have been pinned. It was a large hold and the way it was being used as a foothold meant that pressure was potentially applied in a way that would spin the hold anti-clockwise, therefore making it likely that the hold could spin.
