Thanks for your query. In order to respect the privacy of the participants, could you change the report to anonymise it more? Perhaps remove the family team aspect, since that is not a contributory factor to the lessons.

With respect to the lessons that can be drawn, do you think that there is anything different that could be done if presented with the same situation again?
